At AMCAL Family Services, we are committed to protecting the privacy and security of the personal information of individuals who utilize our services in accordance with Québec's Act Respecting the Protection of Personal Information in the Private Sector (Law 25). This Privacy Policy outlines how we collect, use, disclose and protect your personal information. By accessing or using our services, you agree to the terms outlined in this policy. 

 

Collection of Personal Information 

We may collect information from individuals in the following ways:

  • When individuals access any of our programs or services. 

  • When individuals voluntarily provide information to us, such as during intake assessments, registration forms or correspondence.

  • From third parties with consent, such as referring agencies or healthcare professionals. 

 

The type of personal information we may collect include but are not limited to:

  • Name, address and contact information

  • Health information relevant to the services provided

  • Demographic information

  • Payment information (if application)

  • Information about family dynamics and relationships 

 

Use of Personal Information

We use personal information for the following purposes, including but not limited to:

  • Providing and managing our programs and services.

  • Communicating with individuals regarding scheduling, program updates, meetings and other relevant information.

  • Assessing and addressing the needs of individuals accessing our programs. 

  • Processing payments and invoices (if applicable).

  • Evaluating and improving the quality of our services and programs. 

  • Complying with legal, professional and regulatory requirements. 

 

Disclosure of Personal Information

We may disclose personal information in the following circumstances:

  • To third-party service providers who assist us in provide services (e.g., contracted counselors, support staff).

  • When required or permitted by law, including in response to a court order  or legal process. 

  • With consent, to collaborating agencies or healthcare professionals involved in an individual’s care.

  • To protect our rights, property or safety, or the rights, property or safety of others. 

 

Data Security

We implement security measures to protect personal information against unauthorized access, disclose or misuse. These measures include physical, technical and administrative safeguards.

Access and Correction Rights

To exercise these rights, individuals may contact the program coordinator associated with the services they have used or are currently using, or the Privacy Officer at lfilice@amcal.ca.

 

Data Retention and Destruction 

We retain personal information for the duration of an individual's use of our services and for a period of 7 years after the file is closed, unless a longer retention period is required or permitted by law. After this period, personal information is securely disposed of in accordance with our internal data retention policies. 

 

Some of our measures to securely dispose of personal information include shredding physical documents containing personal information and securely deleting electronic files from our systems. These practices are designed to prevent unauthorized access to personal information and to protect the privacy and confidentiality of individual’s data. 

 

Breach Notification

In the event of a breach of security safeguards involving personal information, where the breach creates a risk of harm to individuals, AMCAL Family Services will notify affected individuals and the Commission d’accès à l’information du Québec in accordance with legal requirements.
